With Archstone Foundation support, California Health Policy Strategies (CalHPS) is leading a CalAIM Statewide Older Adult Collaborative (formerly the CalAIM Statewide Dementia Care Learning Collaborative) to support community-based organizations serving Medi-Cal eligible older adults at high risk of institutionalization and in need of long-term care.
