In early October, two Archstone Foundation staff members visited Choice in Aging, a recent capacity building grantee that provides community-based long-term services and supports to more than 2,000 people in Contra Costa County and the surrounding area.

At its Mt. Diablo Adult Day Health Center in Pleasant Hill, we got to see those programs in action, including multicultural programming for speakers of Russian, Farsi and Dari and programming in several languages for people with Alzheimer’s disease in its middle and late stages. The staff warmly welcomed me and Vice President of Programs Laura Rath and introduced us to participants knitting, dancing to live music, and getting their makeup done for that afternoon’s special program, a Spooky Prom!

With its capacity building grant from Archstone Foundation, Choice in Aging has upgraded its multipurpose senior services program’s case management system to a cloud-based platform, which better serves its 350 participants, and created streamlined processes for training staff to use the system and analyze participant data.

They augmented our support for the project through the state’s Providing Access and Transforming Health (PATH) program, which helps community-based organizations, health care providers, local governments and others make the most of the new benefits of CalAIM, the recent expansion of Medi-Cal, the state’s Medicaid program. Specifically, Choice in Aging received technical assistance via the PATH TA Marketplace and capacity building funding via PATH CITED (Capacity and Infrastructure, Transition, Expansion, and Development).
