A Strategic Focus on Integrated Care
Healthcare, social services, and related systems can and should work together to provide better care.
A focus on team care, training, and technology promotes high-quality care for older adults, reduces racial and ethnic disparities, and furthers Archstone Foundation’s mission — improving the health and well-being of older adults in California.
In addition, we will ensure that these integrated services are culturally competent, client-centered, comprehensive, collaborative, coordinated, connected, and compassionate.
Integrated Strategies to Create Better Health Outcomes for California's Older Adults
These strategies are part of a larger whole, working in tandem to create better health outcomes for California’s older adults.
Care coordination will offer more, and better-connected, resources to populations that historically have had fewer resources available to them.
