The CalAIM program offers important opportunities to address social and environmental drivers of health through Enhanced Care Management and Community Supports.
A key component of successful implementation includes ensuring that eligible individuals are connected to and informed of these services. Area Agencies on Aging, Aging and Disability Resource Connections, California Caregiver Resource Centers, and other information and referral organizations can be key to guiding beneficiaries towards enrollment, especially for individuals at risk for long-term care, high utilizers, and persons living with dementia.
