Data exchange is crucial to coordinating care and services for older people, particularly those who have an array of health and social needs and require multidisciplinary teams to manage care and transitions between settings. Too often, however, health care and social services are siloed, which creates barriers that limit the data sharing needed to improve care and outcomes.

Archstone Foundation and Manatt Health will host a webinar on April 6 previewing findings of a report on the state of data exchange – and the gaps and opportunities to better support care for older Californians.

Panelists will include Eric Hazzard, senior director of the Community Care Hub at Partners in Care Foundation, and Terri Shaw, a consultant with the California Department of Aging. We look forward to sharing the findings and discussing Archstone Foundation’s Technology grantmaking strategies.
