Addressing the social determinants of health has become a major focus of healthcare organizations, which are seeking how best to partner with community-based organizations that will impact outcomes for patients of all ages. The reality is health systems, payers, social services, and community-based organizations must learn how to come together to address social and behavioral determinants of health.

This webinar from Grantmakers in Health, sponsored by Archstone Foundation, highlights key recommendations from the National Academies report, “Integrating Social Care into the Delivery of Health Care: Moving Upstream to Improve the Nation’s Health” and discusses how social services can, and in some settings are, being integrated into clinical care, the infrastructure needed to make it happen, and how foundations might play a role.
