The Archstone Foundation Award for Excellence in Program Innovation was presented to the Diabetes Care Clinics at the University of the Pacific, School of Pharmacy at the American Public Health Association Annual Meeting in October. The Diabetes Care Clinics serve older adults through comprehensive patient education, lifestyle change strategies, and diabetes management. The clinic works with partners in underserved and underrepresented areas of the San Joaquin Valley region, which has the third largest prevalence of diabetes of any county in California. The program engages bilingual/bicultural pharmacy, social work, and nursing students, community volunteers, and staff to foster interprofessional collaboration.
The Archstone Foundation Award for Excellence in Program Innovation is an annual award, established in 1997 through an endowment, to recognize best practice models in the field of aging and public health. By providing national recognition to these programs, Archstone Foundation hopes that it will enable these model programs to be replicated, and to continue to be evaluated in an effort to enhance innovative services for older adults throughout the U.S.
