Join us for a one-hour webinar sharing key results from Improving Access to Quality Dementia Care for California’s Aging Population, a three-year pilot conducted by the Alzheimer’s Association.
The project focused on helping FQHCs and other community-based primary care providers to strengthen dementia care. The external evaluation concluded that nearly all participants achieved some improvements and the majority engaged in robust, durable, or ongoing activities to improve dementia care, such as training, EHR modifications, and quality improvement projects.
In this webinar we will highlight what worked, challenges, and strategies that could make future similar projects even more successful. We will review how lessons learned from this project are shaping statewide and national efforts, including the Alzheimer’s Association’s expanding Brain Health and Quality Care initiative.
