Older adults are challenged by a health care system that is poorly designed to meet their needs and promote their health, well-being, and independence. Health and social services operate in silos and too often fail to connect, plan, and work together. Medical, and public health providers are not well connected to programs and services that address social determinants of health such as housing, food insecurity, transportation, and public safety, among others. In addition, structural racism, social inequities, and a lack of access and cultural competence make it even harder, particularly for older Black, Indigenous, and People of Color (BIPOC) in lower-income communities.
With support and direction, we believe that health care and social service providers can come together to better plan and implement new and innovative approaches that will benefit all older Californians.
